Somehow I doubt we would have so many duplicates if it was only
triggered by a system crash... I wonder if the disconnect between's
APT's view and dpkg's view doesn't come from some event that is not
reported via the status fd (or reported but ignored/missed by APT).

Or maybe dpkg itself being stupid... it could get a request to configure
several packages, but the process of configuring one in facts configures
several because the other were only waiting for the trigger processing
and when it tries to configure the others, it complains because they are
already configured.
