Here http://www.linuxforums.org/forum/gaming-multimedia-entertainment/168732-solved-no-sound-alsa-except-microphone.html someone seems to have solved the problem by building new sound modules from source provided by Realtek: http://www.realtek.com.tw/downloads/downloadsView.aspx?Langid=1&PNid=14&PFid=24&Level=4&Conn=3&DownTypeID=3&GetDown=false When I tried to follow that however, I ended up with sound modules that wouldn't load because of "unknown symbol"s.
