Public bug reported:

Binary package hint: ubiquity

While installing Ubuntu 11.04 from a USB flash drive, the installation
process crashed while attempting to configure the apt sources. The
install was on a freshly formatted HDD.

The error message returned from apt was: "An attempt to configure apt to
install additional packages from the CD failed."

The installer then crashes. Subsequent attempts to install have all
failed thus far.
