I solved this problem recently, the problem occurred (in my case)
because I removed shortcuts manually via the "menu" option, in the
"other" tab. In order to have those needed shortcuts back, I went to
~/.local/share/applications/ and removed *.desktop (I don't know exactly
which *.desktop caused the problem, though). Doing this, all user
added/removed shortcuts revert to original state.

The problem here is that Unity does not even use this menu except for
"open with" menu... so it's weird that it affects unity at a point it
does not even start anymore.
