The report was filed after the error occurred, so USB 2.0 devices had already 
been shut down. The crash in comment #2 followed shortly after. Notice in 
dmesg.txt:
[122397.871035] ehci_hcd 0000:00:10.4: fatal error

Attached is output from "sudo lsusb -v" after a fresh boot when USB
devices are functioning correctly. (Same configuration except that the
cell phone is no longer connected.)
