I've had this happen several times but there is no obvious trigger. In fact it's hard to even tell how long it has been stuck open since I'm doing *actual work* and only notice when the very left part of some window is not visible.
My install was upgraded from maverick (and lucid I think) and is using integrated nvidia graphics.
