After some testing, I have found a possible cause. The instance with SMP and CentOS i386 seems to boot fine when using a different clocksource.
You can test this by booting the CD/DVD image with: linux clocksource=acpi_pm I have tested the KVM clocksource and the acpi_pm clocksource several times and the KVM one always seems to crash. The acpi_pm one always works.
