Setting up firmware-b43legacy-installer (4.178.10.4-5) ... Not supported card here (PCI id 14e4:4315 14e4:1713)! Use b43 firmware. This is just for the b43legacy driver.

Why where you removing firmware-b43-lpphy-installer? In the "binary packages" section in https://launchpad.net/ubuntu/+source/b43-fwcutter/1:013-3 you can see the supported chipsets of each package built from b43-fwcutter. Remove firmware-b43legacy-installer and install the correct package (you may need to reboot your computer).
