OK, I'd like confirmation from AJenbo on the following:

1- were you running from a USB bootable stick, or from a hard-disk
installation of Ubuntu?

2- What happens if you insert the same USB thumb drive that failed and run this 
on a terminal:
/usr/share/checkbox/scripts/usb_test -t

3- Could you insert the USB stick and post the output of running this on a 
terminal:
mount
