Public bug reported:

Hello,

I had some problems trying formating my hard disk with windows 7 and I
have decided to install the last edition of ubuntu to avoid the annoying
windows things. I made a usb disk for the installation. When I first
entered I got a hard disk failure problem and I cannot even run the
examine program. When I started the format process and the installation
of the ubuntu, I got a failure message saying that the hard disk
crushed during the installation. Anyone knows what I have to do?
