Thanks for reporting this bug. First, let's leave the other problems (pulseaudio, rhythmbox etc) for the moment, a kernel shouldn't OOPS no matter what you throw at it, and it is probably a separate problem anyway. Also; I tried powercycling the USB midi stuff I have here, and it seems to work fine, so not reproducable for me.
When looking at the oops and the source code, it seems introduced by commit 88a8516a2128 (implement USB autosuspend), but difficult to know for certain.
