For me this happens when I try to open Pidgin from the top right letter icon. 
What happens is that Pidgin kind of starts, but is invisible, the Pidgin icon 
is visible with the arrow in the Unity pane. To get Pidgin visible I do:
1) right click the pidgin icon in the Unity pane -> click "Quit"
2) reopen pidgin from the top right corner letter icon
3) pidgin is again invisible, press alt + tab to "change window" (although it 
is not necessary to have any other applications open)
4) click pidgin icon from the Unity pane, now it should be visible

This happens also with automatic updates, the update window is invisible
unless I "change window" with alt+tab. It has something to do with
window focus or something. The problem also appears in Firefox as having
invisible context menus, the alt+tab thing works for these too.
Sometimes the Firefox works, sometimes not. I use a lot of virtual
desktops and it seems that changin the desktop somehow breaks the window
focusing. The context menus usually flash quickly and get invisible, as
if they were hidden behind the fullscreen Firefox. Also Firefox address
bar and every place on a web page where you can write text and Firefox
shows its suggestions, have this problem.
