Getting: /dev/dm-1: read failed after 0 of 4096 at 0: Input/output error

/boot is sda2 and 
/ (root) is on lvm (/dev/VG/rootfs = lvm sda6) 

Reason could be with group-mount trying to mount lvm snapshot

Removing that snapshot and running update-grub and dpkg-reconfigure
memtest86+ everthing works
