I experience the 2nd problem you mentioned except I use ndiswrapper. My chipset is bcm43xx. I am able to connect to wireless networks when this problem arises with network manager by command line (ifdown eth1 && ifup eth1). This is why I think it's a problem with network manager. But, seeing your comments and modifications to this bug I beginning to wonder if we talking about the same bug.
