Public bug reported:

After several failures to upgrade from 11.10 to 12.04.  This attempt to
reinstall 11.04/11,10 seems to have been partially successful.  OS
installed to HDD2 instead of HDD1 (re-partioning under 12.04 install
seems to have left some /ext files.
