This happened here too with my "Audio device: Intel Corporation 82801I
(ICH9 Family) HD Audio Controller (rev 03)" but it's a bit more
complicated.

I added a second (usb) audio device and it showed up in the hardware
list, but when I selected it the input and output device lists didn't
change (they stayed set as the internal device).

So I set the internal one to Off which switched the output device to
"Dummy output" but the usb input and output devices still weren't
listed.

I then removed the usb device and set the internal one back  (Analog
Stereo Duplex) but the output device still said "Dummy Output". Logging
in and out didn't fix this.

After reading this bug report, I sent a TERM signal to the pulseaudio
daemon and it restarted and now it works fine. I can switch the profile
to off and back and it works fine without killing it. And I can also
switch to my usb device and back and it works as expected.

Seems a bit like pulseaudio wasn't properly responding (fyi, the
pulseaudio process was over 10 days old and didn't restart after a login
and logout (i.e: the process wasn't killed when I logged out).
