We removed all non main ubuntu repositories from sources.list and updated apt, 
then removed packages not in repositories with the list that we get from:
apt-show-versions | grep 'No available'

Without rebooting, we do a 'do-release-upgrade -d' and it worked
perfectly... so, our problem was a mix of ubuntu repositories in
sources.list.
