This bug is somewhat difficult to reproduce. I still see it from time to time, but it seems to only happen under certain conditions, for example one process is maxing out a single CPU core, and a different one is trying to do a lot of I/O. It also seems to depend on the USB device, possibly only affecting certain transfer sizes. It's not as simple as just plugging in a thumb drive and dd'ing to it (although that is the primary use case when I see the bug.)
Additionally since I reported this bug I've upgraded my memory to 16GB. No, the issue was not caused by swapping, but the extra memory really cuts down on the i/o pressure because now pretty much everything is permanently cached.
