I am experiencing similar issues with 12.04 Unity. Though I am not sure when/how it occurs, certainly not the 'click top left icon' jig But I have noticed most of the times after lock/unlock, the windows borders are gone.
I tried 'unity --reset' once but that didn't seem to resolve this. Please let me know what all information to collect for debugging this and I'll get it done.
