To reproduce you will need:
USB headphone/mic
Pavucontrol
gonme-control-center/sound
Rhythmbox

Steps to reproduce:
Open Rhythmbox, select a track
Plug in the headset
set the headset as the default device
Unplug the headset
reboot
Open the Sound settings and pavucontrol
start the track playing in Rhythmbox again
plug in and unplug the headset.

Results:
Gnome-control-center/sound doesn't update that it is using the headphones
Pavucontrol updates correctly

Expected results:
I expect both to give an acurrate feedback of which device is being used.
