I experienced this issue on a fresh 12.04 install on a Lenovo Ideacentre K330 with the Intel i3-2120 3.30GHz CPU.
The issue resolved after running the commands listed in Step 1 at https://help.ubuntu.com/community/SoundTroubleshootingProcedure#Step.
