I am also experiencing this issue simply by playing back Youtube video straight after doing a cold boot in 12.04. It starts happening pretty soon after the video has started playing back. I'm not playing anything over the network, just locally on my machine. Unfortunately I'm not at the machine in question at the moment but can provide a more thorough lspci dump plus any logs later if needed.
