Precise 12.04 same issue.

Following the first 'Safely Remove':
$ dmesg | tail

[16817.664123] usb 1-3: USB disconnect, device number 3

It appears that the card reader port is powered off. Pulling the SD card
and inserting into another system works - card opens right up.
