One of these messages is not unusual; line 206 of the script calls activate_vg to try to create the device node in the case that the source device is on LVM. The second message, however, seems to come from line 304, and I don't know why that's being hit. I think you should edit the script to add more debugging information. You may also want to change the message() function to *not* use plymouth, since plymouth messages will only show one at a time, making it difficult to track what's happening. You may also want to check if this problem is reproducible with the initramfs from the quantal kernel.
