I have encountered the same issue, it is present with the original LibreOffice on quantal and also with the updated version from the PPA "libreoffice-libreoffice-4-0-quantal".
I will upload the affected document (almost empty, I was not able to get far) and some crash reports (not sure this are the correct files). To reproduce the issue try editing a formula in the document.
