Public bug reported:

flashcache_destroy always seems to fail. Does it never work for cacheBACK 
devices? Am I doing something funny? Here's a dump of a session where I play 
around with lvm and flashcache attached.
I can only seem to remove the flashcachedev using 'dmsetup remove'.
