Ubuntu 12.04

Sound was fine earlier this morning, but is now at so low a volume as to
be almost inaudible. I didn't make changes to any config items or do
anything else. The only piece of software I started between sound being
fine and it being too quiet was "anki" - a flashcard app. Rebooting made
no change.

Gnome, KDE, and alsamixer are all set to output at max volume. The
problem affects all applications.
