Finally I have a means of reproducing this reliably and feel like I can
file a bug.

This may be hardware dependant. The driver concerned is snd_hda_intel,
device is

00:1b.0 Audio device: Intel Corporation 6 Series/C200 Series Chipset
Family High Definition Audio Controller (rev 05)

Reproduce :

* Play music in Rhythmbox
* While music is playing, visit http://www.maiagame.com/
* Audio playback breaks horribly with lots of high pitched interference and 
stuttering
* Closing browser does not fix this condition
* Pausing music does not fix this condition
* `killall pulseaudio` fixes this condition

Any apps with an open audio stream will not reconnect to the new
pulseaudio process,  (Rhythmbox reconnects for next track , or have to
restart the app for video players, reload the page for YouTube, etc).

Other ways of producing this state include seeking at random in video
(by using slider bars in either Totem or VLC), playing certain Flash
videos, etc, but this is the first time I've come across an audio source
that reliably produced this effect ; possibly something to do with its
combination of short high pitched intermittent tones.
