I'm using Firefox 22.0 on Fedora 19.  I tried to log into a site typing
a lowercase name.  Auto completion offered both the lower case and
initial caps version.  But nothing I tried would let me get the
lowercase name (tab, mouse click (on choices, or on other part of form),
arrow key).

Jesse on #firefox suggested that I edit the saved passwords.  I found
that there was an entry for each casing.  I deleted the initial caps
version.  Then all was well.

It seems odd to me that even though both spellings were in the table. I
could only access one spelling.
