You *did* have an /etc/default/grub at the time; it just contained some
trash.  There is a difference between what is in your MBR and what
packages you have installed.  You had the grub-pc package installed,
which provides the file /etc/kernel/postinst.d/zz-update-grub, which
causes the kernel to update it when it is upgraded.
