Hi, It's hard to tell why PulseAudio crashes or does not start. You do have some worrying messages about read failures from sda (which is usually your hard drive), but I'm not sure if that's related.
The best would be if you could also produce a PulseAudio log: https://wiki.ubuntu.com/PulseAudio/Log - and try to connect to HDMI while the log is running. Can you do that?
