I've since upgraded to the 3.16.0-29 kernel. Bluetooth still doesn't work with it, but my patch still fixes it. Upon a power cycle, ath3k did not load automatically, but with a modprobe it loaded and worked. Running depmod fixed that too. I also tried the 3.13.11-ckt13-trusty kernel you posted. The bug exists in it.
Jamie (jgwild), your Bluetooth chip has a different USB V/PID, different Bluetooth chip manufacturer, and the problem causes entirely different symptoms (detected, but non-functional / not even detected).
