I had the same issues some time ago with Azureus and Firefox. The "always use this app for this kind of files" checkbox was on, but I always had to click OK when downloading something. I'm not sure whose fault this is, some sites don't provide the user with a full http:// link to the file being downloaded, but generate them in the background somehow and pass that to the browser afterwards.
If I described your problem well, this bug could be set to confirmed.
