14.04.2: inserted an ISO image with Hyper-V manager, but VM did not see
it.

Mounted it manually:
# mount /dev/cdrom /media
mount: block device /dev/sr0 is write-protected, mounting read-only
# df
...
/dev/sr0     55548     55548     0     100%     /media

Looks good. Unmounted via hyper-v manager.

No new messages, no entry in /var/log/syslog, no change in df output,
cached directory listing still cached.

Hyper-V manager shows ISO as ejected, but am still able to look at files
on virtual CD.

Kernel 3.16.0-34-generic

Is this only fixed in 14.10?
