Public bug reported:

Please ignore bug report #1466803 as it is inaccurate - this replaces
it.

The Setup:
VirtualBox 2 x 8GB disks
Software RAID 1 or 10:
md0 = 1GB /boot bootable
md1 = 7GB LVM (1GB swap, 6GB root) - encrypted for (1) below so the initial
setup looks like...

for test 1: Software RAID > encrypted volume > LVM
for test 2: Software RAID > LVM

The issues (tested on VirtualBox with and on 14.04.2):

1. Boot of encrypted LVM RAID system hangs when disk is removed.
Last boot messages are:
Reading all physical volumes. This may take a while...
No volume groups found
No volume groups found
Begin: waiting for encrypted source device... ...

2. Boot of UNencrypted LVM RAID system hangs when disk is removed and replaced.
Last boot messages are repeated:
Incrementally starting RAID arrays...
mdadm: CREATE user root not found
mdadm: CREATE group disk not found
Incrementally started RAID arrays.

** Affects: mdadm (Ubuntu)
     Importance: Undecided
         Status: New
