This issue seems to have been solved by one of the recent updates. I hadn't changed anything in my settings for the past week, but this morning decided to redo `dpkg-reconfigure server-xorg`. I then rebooted and no longer got the initial message that it was defaulting to failsafe mode - and now my settings appear to work correctly.
