re disabled auto mute, the problem was detected while auto mute was enabled.  I 
only disabled it while experimenting with alsamixer, and did not change it back 
because it did not affect my user experience at all.
Perhaps it is relevant to know that I keep a set of old self powered computer 
speakers permanently connected to the rear line output jack, and control 
whether they make noise or not by altering the levels on the speakers - so 
afaik the computer has no awareness of whether they are making noise or not, 
they will just always appear connected.

Would it be useful if I test again and/or regenerate files with auto
mute enabled?
