Matt, I think it's fine that upstream has demoted the BUG_ON, as I haven't heard anyone report this with a kernel later than 3.13; I assume whatever is causing it is fixed in later kernels.
At this point there's not much more I can do, as I can't reproduce it and don't have much debug info on exactly what/why the zone's pfn range becomes incorrect.
