Same problem with plain ext4 root partition. I have two systems
configured very similarly with 16.04, one has this problem and another
has not. But their hardware is different, so I'd make an educated guess
that this is a timing issue. Good thing is that it appears benign.

BTW, I get three errors:
/scripts/init-premount/plymouth: line 38: /sbin/plymouthd: not found
/scripts/init-premount/plymouth: line 38: /bin/plymouth: not found
[fsck displays its output here]
/scripts/init-bottom/plymouth: line 18: /bin/plymouth: not found
