Same on Ubuntu MATE 16.04 (as probably on any flavor). AFAIK the issue was brought about by changes in the behavior of the Save button: Earlier LibreOffice versions (such as the one included in 15.10) would gray out the change button when there were no unsaved changes and enable it as soon as changes were made. The icon was the same in both cases.
Somewhere in the recent versions (Xenial currently has 5.1.2.2) this was changed: the Save button is always enabled, and the icon changes depending on the presence of unsaved changes. With no usaved changes, the "old" icon is used (for which the Human theme has a replacement). When changes are made, the icon changes to a new one, which, presumably, Human doesn't know about yet, resulting in the Galaxy icon being used as a fallback.
