Kyle could you please do the following:

boot with root=/dev/md0

check if the UUID used by grub is the same as the one in /dev/disks/by-uuid
(the symlink should point to ../../md0)

If it is not the same UUID, fix it in /boot/grub/menu.lst

update the initramfs or make sure to have the latest mdamd installed
(ubuntu3).

reboot again.

This fix did work for basically everybody reporting a similar issue, the
only thing i can think of is an inconsistency on your system.
