Apologies for the double comment - but I realized I missed one of your questions. The size and type of files does not seem to matter. I've had this happen with text, image, video, and "other" files of sizes from a couple kilobytes up to several megabytes. I have not tested with a large number of files of several gigabytes, though I don't imagine it would be as noticeable since one would reasonably expect those to take a bit of time to copy normally.
As far as I can tell there are no definite triggers. When I was first doing a prototype build of Ubuntu 16.04 LTS and ran into this bug, I did a fresh build, updated all packages, and then was able to easily replicate after a clean boot and nothing running except the file browser.
