Public bug reported:

I had to manually created mirrored disks, and I am using UEFI boot.

I have 
/dev/sda1 = 128M biosboot, 
/dev/sda2 = efi, 
/dev/sda3 + /dev/sdb3 = md3 = /boot, 
/dev/sda4 + /dev/sdb4 = md4 = /root

It failed trying to install grub.
