Public bug reported:

I tried to install ubuntu many times but it always fails with error
number 5 input output error

The installation was done via USB stick prepared with Startup Disk
Creator. I tried also with unetbootin, same error.

I tried again formatting the host hard disk via gparted (multiple times) but 
the error is still there.
I was not happening time ago while installing on the same disk ubuntu 14 (now I 
am installing 16.04.3)

I tried also a dsck in the host disk (/dev/nvm...) but it does not seem
to show errors

The disk is a samsung 960 EVO Pcie Nvme
