Joseph, Please minimally pull in patches 4/6 and 5/6 from the following upstream submit to address this regression. Apologies for this ongoing hassle.
The 12.0.0.4 patch set was pushed upstream. The patches 4 and 5 are the minimum that should be applied to fix this issue. [PATCH 4/6] lpfc: Fix 16gb hbas failing cq create.
