I have seen evince hold on to the audio device - Though I have only seen it after evince has been launched by Firefox - unfortunately it seems that Firefox does not close file descriptors when it spawns helper apps - hence if firefox has the audio dev open (quite often the case - e.g. after using Flash etc) then evince inherits the open audio descriptor and hangs onto it - even after Firefox is closed. In fact it happens with any app started by Firefox.
