So there seem to be at least two problems here:
1) People who never get window decorations (titlebars)
2) People who normally have titlebars, but they disappear after some trigger.

(1) Is probably caused when a required feature is not supported by the
driver/card/Xorg.conf combination.  For example, (some) nvidia users
require AddARGBGLXVisuals "true" in their xorg.conf in order to get the
enable the alpha channel in OpenGL visuals - required for gtk-window-
decorator (the Restricted Manager automatically adds this option, but I
guess this could be biting upgraders or people manually installing the
nVidia drivers).

(2) Would be caused by gtk-window-decorator crashing.  Can someone who
experiences this grab a backtrace of the gtk-window-decorator process,
as per <https://wiki.ubuntu.com/Backtrace>?
