Here's what I have:

I already tried completely uninstalling everything - all the ubuntu restricted 
packages, and the nvidia-glx package, and then using the nvidia-installer 
script in "uninstall" mode
so as to be sure of doing a clean install each time.

The driver from nvidia.com, (or, equivalently, using envy) always works;
the ubuntu packaged one always fails.

Incidentally, I administrate 3 other machines, all using 64-bit Gutsy,
on similar processors, and with similar Nvidia 7600 GT cards. These work
(and always worked) just fine with Ubuntu's package. The cards are
slightly different though - my desktop (in this bug report) uses a 7600
GT which has 2 DVI ports (on dual monitors), whereas the other machines
have 7600-based cards which are only capable of driving a single DVI
output.
