I was unable to reproduce this failure on either a xenial minimal or
standard image. I went ahead and ran through the testing documented in
comment #15 with no issue. Each time, the device file was present as
expected and the hv-kvp-daemon.service was running normally. Even though
I couldn't reproduce the issue as originally discovered, I'm marking
this as verification-done-xenial as the problem no longer occurs with
the 4.15.0-1067-azure kernel.
