16.04 LTS is supported until April 2020 (maintenance), so it's
recommended to use 18.04 LTS (18.10 not being LTS and short in support,
see : https://ubuntu.com/about/release-cycle ).

This bug does not apply to fresh installs (although it may be related) while 
wiping the disk, see initial post.
Make sure to check your BIOS settings (EFI/non-EFI, boot sector protection) and 
please post a new full bug report with full details if you have issues ( I 
suggest on 18.04 LTS).
